The FC Northwest U16 Boys Soccer Team would like to thank you once again for your contribution to our trip to Charlottetown, Prince Edward Island, where we represented Manitoba at Canada's preeminent soccer tournament.
The total cost of the trip was in excess of $30,000 and your consideration was helpful in ensuring that players and coaches were able to travel to this prestigious national sporting event.

WOJCIK'S FUNERAL CHAPEL ENTRUSTED TO CODY JAMES SOUTHALL
It is with deepest sorrow that we announce the passing of our son, Cody James Southall. He passed away peacefully in his sleep at home of a Pulmonary Edema on Thursday, November 1, 2012 at the age of 11 years.
Left to forever cherish Cody’s memory are Papa (Patrick); Mama (Marie); big brother Bailey; Nana (Sharon); Baba (Jennie); Grandpa Doug and Grandma Betty; Uncle Kris, Rachel & Caitlin; Auntie Monique & Dave; Uncle Peter & Tyson; as well as numerous other Aunts; Uncles; cousins and friends.
Cody lived with Autism, Seizure Disorder and Heart Arrhythmia. He loved swimming, going on bike rides, walks to get slurpees, sitting on his swings, camping with his family, watching movies, Thomas the Train and being in the boat with his Papa.
On Friday, November 9, 2012 a Celebration of Cody’s Life will be held at 7:00 p.m. in the Bord-Aire Community Centre, 471 Hampton Street, Winnipeg.
A huge thank you is extended to all of our family and friends who came to be with us over the past couple of days. A special thank you to Uncle Kris for being our pillar of strength during a very difficult time.
We have decided that we will be opening a trust fund in Cody’s honour so that we can help other families who are raising children like Cody. Donations to this cause would be greatly appreciated in lieu of flowers. Donations can be made to Pat or Marie Southall.

Preparing for an Expected Death at Home
FAMILIES AND PHYSICIANS EXPECTING A DEATH AT HOME
The following information is helpful for physicians and families on the procedures to follow to facilitate the expected death of a patient in the community. Specific direction should be given to the family regarding the notification which must take place after the death occurs, and regarding the arrangements to be made for transportation of the deceased. If these directions are followed, the confusion that can occur if Police or Ambulances are called can be avoided.
The family must advise the physician of one of Wojcik’s Funeral Chapels that they will be using after the death occurs.
The physician must write a letter to Wojcik’s Funeral Chapel & Crematorium to advise them of the pending death and who will be responsible for the Registration of Death. A suggested draft form of such a letter is included. Clearly state any specific precautions about which the funeral home should be aware, e.g. “pacemaker”, “observe body fluid precautions”. This letter authorizes Wojcik’s Funeral Chapel & Crematorium to transfer the deceased and complete Part I of Registration of Death.
The letter must be sent to Wojcik’s Funeral Chapel & Crematorium with copies to the family and to the Office of The Chief Medical Examiner.
When a death occurs at the family home, DO NOT phone “911” (ambulance or police). Call Wojcik’s Funeral Chapel & Crematorium to transfer the deceased and they will also advise the attending physician who will be responsible for completing the Registration of Death.
If the family calls “911”, resuscitation efforts can be avoided if they produce the letter of expected death for the ambulance personnel. At this point the ambulance will leave and the family should notify Wojcik’s Funeral Chapel & Crematorium and the physician. Families should take as much time as they feel they need prior to calling Wojcik’s as will normally arrive within one hour from a family calling us.
Then Wojcik’s Funeral Chapel & Crematorium and/or the physician must notify the Chief Medical Examiner’s Office that the death has occurred.

Richard Wojcik and Staff: Thank You from the Smith Family
Dear Richard, Louise & Staff,
Mere words can never describe what I feel so deep within for all you have done for me since Jack passed away September 11, 2012 until the end of his memorial service September 23, 2012.
I'll never be deserving of your kindness, understanding, patience with me, your hours of work, being so accommodating and going above & beyond in every way for me but if anyone is truly grateful & appreciative, I certainly am forever.
I'm comforted that you'll be there for our daughter Darlene when my time comes. The guest book, laminated obituary notice, thank you cards, your warm letter are cherished gifts/keepsakes that give me strength & comfort. A huge part of me died with Jack. I loved and shared 38 years of my life with Jack. You made his send off possible & wonderful my heartfelt thanks.
